Discharge Care


Prior to Discharge

We want to be sure you fully understand your discharge summary.  Before you leave the hospital, discuss any questions or concerns with your hospitalist regarding:

  • Medications or reactions that may occur by taking the medications
  • Services you feel you may need outside of the hospital (example: home health care, if you are unable to care of yourself when discharged)
  • Excuses related to work or school you need to return to your daily activities

Follow-up Care after Discharge

Hospitalists at The Ohio State University Medical Center are only able to care for patients while they are in the hospital.  Our goal is to coordinate your follow-up care with your primary care physician and any other physicians involved in your overall healthcare.

Once you are discharged, please follow-up with all necessary office appointments you have and/or need to schedule. Any questions or future appointments after discharge should be directed to your Primary Care Physician.  Click here to find a Primary Care Physician if you do not already have one.

Please note: appointments made during your hospital stay are listed on your discharge instructions discussed with and given to you prior to leaving the hospital.

Prescription Refills and Long-term Care

Hospitalists do not routinely write prescription refills. You will need to follow up with you doctor (Primary Care Physician and/or other specialists) for any refill orders and long term care once you have been discharged.
