2009 - 2012 , MS4 - Quality Improvement Elective - I serve as an advisor to 4th year Medical Student on their quality improvement projects. Projects have included:



1.Increasing the delivery of Shingles vaccinations to the adult population at the General Internal Medicine Clinic at Morehouse. Lessons learned from this 3 month QI study formed the basis for a randomized study completed in 2011, manuscript has been submitted for publication.



2. Improving medication reconciliation in the ambulatory setting. The workflows established have become standard practice in the clinic and considered a best practice workflow.



3. Improving guideline compliance and safe medication management in patients with stage 3 and 4 chronic kidney disease using a population based model of care and ancillary pharmacy support.

Partnership Narrative

Healthy at Home Columbus (House Call Program) - I am collaborating with the College of Nursing to establish a visiting nurse/physician program to serve the frail elderly in central Ohio. Start date is October 2012. We will manage up to 150 patients providing care in the home using mobile technology to bring electronic medical records into the community. Funding support is being provided by the department of internal medicine and the medical center. The program will serve as a training opportunity for the nurse practitioner students as well as geriatric fellows, internal medicine residents, and medical students.



Our aim to grow the program over the next 5 years with an expansion plan to manage up to 500 patients.



After hours clinic at Morehouse Clinic - I have collaborated with the Emergency Department at OSU Medical Center to establish an after-hours clinic for primary care services in our general internal medicine clinic at the Morehouse Plaza. The clinic will have lengthy evening and weekend hours and provide a much needed service. The aim is to decrease the unnecessary use of expensive emergency room care after hours.



Population Management with OSU Health Plan - I am working with the university health plan to establish population management workflows to maximize the use of generic medications and formulary selection for plan members.
