Team Based/Patient Centered Care for Youth and Young Adults with HIV

Health Care Transitions for Youth and Youth Adults with Complex, Chronic Medical Conditions

I completed my residency training in Internal Medicine-Pediatrics in 2003 at Christiana Care Health Systems (now Thomas Jefferson University/DuPont). After completion, I was a staff physician at the Health Care Center at Christiana in the Med-Peds Faculty Practice and was a preceptor in the Med-Peds Residency Continuity Clinic. In 2006, I moved back to Columbus, OH (where I am originally born and raised) and began working at Nationwide Children’s Hospital and The Ohio State University. I currently split my clinical and academic time between medical education at the medical school, the FACES HIV clinic at Nationwide Children’s, resident precepting at the South High Primary Care Center and Med-Peds Academic Practice at our Grandview office. I hold a courtesy appointment in the Department of Internal Medicine at The Ohio State University College of Medicine, Wexner Medical Center.

Partnership Narrative

In my work with the HIV program, I collaborate extensively with faculty in the Division of Infectious Diseases in the Department of Pediatrics and Department of Internal Medicine. Since we now provide obstetrical services in our clinic, I have also collaborated with the Division of Maternal-Fetal Medicine in the Department of Obstetrics and Gynecology. My most recently collaboration has been with the Departments of Psychiatry at Wexner Medical Center and Nationwide Children’s, as we expand mental health services for HIV infected patients at FACES and for others in Central Ohio. We have many external partnerships as part of the AIDS Service Organization community in Ohio, including the Ohio Department of Health, Columbus Public Health and other ASOs. As part of our community outreach, FACES has entered into a collaboration with the Neighborhood House, a hub of community based services located in an underserved area in the King/Lincoln neighborhood. We started this collaboration to provide walk in HIV testing in the community at a stable location, but hope to expand our relationship to include mental health and substance abuse services.
