Thank you for your interest in the Ohio State University Division of Cardiovascular Medicine Fellowship Program. Eligibility criteria for application to the program include the following: - MD or DO
- US Citizenship, Permanent Resident, or J1 Visa Status
- Completed 3 years of US postgraduate training by the time of entry into the fellowship program.
For our 3 year core Fellowship Program, we will be accepting applications only through ERAS (Electronic Residency Application Service). Deadline for application to the core program for positions beginning July 1, 2009 is December 15, 2007. Paper applications to the core program will not be reviewed. ERAS applications should include a complete CV, personal statement, three letters of reference, USMLE Board Score Reports as well as a recent photograph. All positions are filled through the NRMP match. Application to the subspecialty programs including Cardiovascular Diagnostic Imaging, Interventional, Electrophysiology and Heart Failure can be made by completing the On-line Application. Deadline for applications is as follows: - Electrophysiology - December 31, 2007 for positions beginning July 1, 2009
- Heart failure - August 1, 2007 for positions beginning July 1, 2008 (NOTE: cardiology fellowship MUST have been completed prior to entering this program)
- Interventional - December 31, 2007 for positions beginning July 1, 2009
- Cardiovascular Diagnostic Imaging - July 1, 2007 for positions beginning July 1, 2008. (NOTE: cardiology fellowship MUST have been completed prior to entering this program)
In addition to the on-line application, the following must be submitted: - Online Application
- Complete CV
- Personal Statement
- Three Letters of Reference
- USMLE Board Score Reports
- Copies of Doctoral Certificate and ECFMG Certificate (if applicable)
- Recent Photograph
