Thank you for your interest in the Ohio State University Division of Cardiovascular Medicine Fellowship Program. Eligibility criteria for application to the program include the following:
- MD or DO
- US Citizenship, Permanent Resident, or J1 Visa Status
- Completed 3 years of US postgraduate training by the time of entry into the fellowship program.
For our 3 year core Fellowship Program and our 1 year Interventional Program, we will be accepting applications only through ERAS (Electronic Residency Application Service). Deadline for application to either program for positions beginning July 1, 2011 is December 15, 2009. Paper applications will not be reviewed. ERAS applications should include a complete CV, personal statement, three letters of reference, USMLE Board Score Reports as well as a recent photograph. All positions in the core program are filled through the NRMP match.
Application to the subspecialty programs including Cardiovascular Diagnostic Imaging, Electrophysiology and Heart Failure can be made by completing the On-line Application. Deadline for applications is as follows:
- Electrophysiology - December 31, 2009 for positions beginning July 1, 2011
- Heart failure - August 1, 2009 for positions beginning July 1, 2010 (NOTE: cardiology fellowship MUST have been completed prior to entering this program)
- Cardiovascular Diagnostic Imaging - December 31, 2009 for positions beginning July 1, 2010. (NOTE: cardiology fellowship MUST have been completed prior to entering this program)
In addition to the on-line application, the following must be submitted:
- Online Application
- Complete CV
- Personal Statement
- Three Letters of Reference
- USMLE Board Score Reports
- Copies of Doctoral Certificate and ECFMG Certificate (if applicable)
- Recent Photograph
The online application should be submitted electronically to the Cardiovascular Medicine Division. The remaining items should be mailed to the Division at the following address.
